I do think its time to rethink/reform our FWII process. I also believe it might be time to have AOTA push back on Genesis and say while we appreciate your support, we do not agree with your practices, and not give them the advertising they receive at the EXPO.
Finally we were notified by our contracts and personnel dept that they will not renew the contract because of the imposed upon fees.

I refuse to sign the amendment.
It is a slippery slope. In my mind, it is their opportunity to generate revenues. Evidence is clear that payment does not improve clinician's productivity or students' clinical experiences.

We recently received an amendment to our contract with GENESIS to include a payment ($1,000 full-time $500 part-time) for student placements. I was wondering how (if at all) it would effect your FW programs and if you have a mechanism for paying the fee (i.e. have you faced this with other sites and paid for student placements).

Lynne,

We received the same amendment. We currently would have no choice but to pass that cost to the student as our budget wouldn't support that expense. But we haven't decided what we will do yet.

I am concerned about this.... If we all comply, will this start a trend? We may not have an option due to the heavy competition for sites? Can we call on practitioners/state associations to push back and help support education citing a professional duty to educate the needed workforce?

More questions than answers today. I'll be interested to hear reactions here and conversation at our meeting this weekend.

Food for thought...ask for reimbursement as a finder's fee if the site or company employees the student within 3-5 years after placement completed

Another concern is FW contract versus now a vendor. What are the legal issues of such a relationship?

That's a great idea. I don't know if Genesis is currently paying sign-on bonuses, but that should come right back to the college if a student does FW and then gets a job with them.

We hear this has been changed by Genesis, and feel that it will change our processes down the road as other sites will likely join in. This is a concern to us. In the near future we will likely not utilize Genesis unless we absolutely have to, choosing other FW sites first. Our university has paid stipends to preceptors in medicine before, but never for OT or PT - not sure how we will handle this yet - but we will place students at other sites as much as we possibly can.

We just had our first request for this in an out of state placement, not yet locally. Can AOTA respond and talk to these these large national companies? What is their rationale? I agree that it is a dangerous trend and could put many of us out of business.

We are facing this as well and do not have a mechanism to pay. A panel of fieldwork supervisors will address at our state conference in November as payment for Level II placement has been announced by three of our current fieldwork contracts (large multi-site entities). Among the suggestions was to discuss with APTA and ASHA to address more broadly.

We received the same notice from Genesis and it has a significant impact on our fieldwork program. I noticed that the topic is on the agenda for our ALC meetings later this week.

We know about this as well and do not use this or any other site that requires payment. If too many organizations started requiring such payment, we would need to give serious consideration to our budget, potentially needing to pass on such costs, at least in part, to students. That would be unfortunately given the already high cost of graduate education.

We at CSM received that notice as well and are not prepared to act on this, nor should we. In my nearly 40 year career, I've seen this issue come up occasionally and it is a sad state to charge students for placements with a particular company. We endeavor to rally the schools in the area to discuss a way to stop this possible trend. It's a slippery slope once it starts. Yolanda Griffiths, OTD, OTR/L, FAOTA

This is becoming an issue for us. There has been a threat of this happening by one of the big hospital systems. They have charged us for onboarding so far but indicated an additional fee for all level 1 and 2 fieldworks. We have talked about adding an additional fee for students but we are hesitant to do so.

We have gotten the MOU amendment as well. I wonder if this will change Genesis's availability for placements because I know they say NO to us all the time at the administrative level even when we have therapists who have said yes.

I agree that this is a trend and problem. It's time for our profession to seriously investigate other possible fieldwork models. I will not be travelling to San Diego, but will be glad to hear what the discussion is about fieldwork. We need to be proactive to help our profession move forward.

This is becoming an issue for us. There has been a threat of this happening by one of the big hospital systems. They have charged us for onboarding so far but indicated an additional fee for all level 1 and 2 fieldworks. We have talked about adding an additional fee for students but we are hesitant to do so.

Thanks

I agree with Nancy that it is time to propose some other fieldwork models. This needs to be an in-depth discussion with potential suggestions for change in the near future.

We will not be able to pay the Genesis fee, so we will no longer be working with this organization for fieldwork opportunities. We had a meeting with Genesis about this change and brought up some additional questions such are: as we will be paying for the fieldwork experience do we have an opportunity to interview and chose the fieldwork supervisor? What if we (or our students) are unhappy with the learning experience? Then do we get our money back from the site?
At the time of our meeting these questions could not be answered.
